ACC is seeking a part-time Nurse to provide community nursing for the Asian Pacific Islander Dementia Care Network (aka Bridge to Healthy Families). This position is responsible for information & assistance and some care management for families affected by dementia. This position (a) assesses the behavioral, social, emotional, and practical care needs of persons with dementia and formulates care plans to address them, (b) provides counseling, education, skills training, and coaching for caregiving families, and (c) identifies community resources and makes referrals to appropriate community resources. The Nurse reports to the Senior Program Manager and works closely with Bridge to Healthy Families staff, other ACC staff, interns, and volunteers.
